Questionnaires

The court’s jury process has two phases. The first phase requires the completion of a Qualifying Questionnaire. The questionnaire is accessible through eJuror and aids the court in identifying persons legally qualified to be summoned for jury service.

The second phase involves summoning previously qualified persons to court for jury service. If you are randomly summoned you will receive a mailed notice directing you to complete additional questions in eJuror. The information you provide will assist litigants and the court in determining whether you should be selected to serve on a jury when you report for service.

eJuror provides safe, easy online access to the Qualifying Questionnaire and the Summons Questionnaire.  These questionnaires are required, even if you are eligible for excuse, exemption or disqualification. If you do not have access to a computer, contact the jury office.

If you received correspondence regarding either a Qualifying Questionnaire or Summons for jury service, please use the link below to log in to eJuror to complete your required questions. Have your letter nearby for reference as you will need your 9-digit participant number near the barcode.

Summoned Jurors
